Complete and submit this form to report updates to employer information during your Optional Practical Training (OPT) authorization period. It is required by law that F-1 students on OPT report changes within 10 calendar days of the change (see 8 C.F.R. § 214.2(f)(12)(i)).

If the below update is for STEM OPT employment, reporting is not complete until your immigration advisor has received all required I-983 updates (a new I-983 for new employers and/or the final evaluation on page 5 of the I-983 for ending employment).
